LPHA is completed by a Licensed Professional of the Healing Arts (LPHA). The therapist will meet with the client to assess symptoms, behaviors, and circumstances. The LPHA will then make a mental health diagnosis and help link the client with a provider and/or therapist for the recommended remedial services community.

 

LPHA’s Assessments

All assessments will be completed by a licensed mental health clinician (LMFT, LMHT, and LISW) with the cooperation of the individual client or parent/guardian. Assessments are completed in a narrative format and contain the following information: 

 

  1. Relevant information regarding the onset of disability and diagnostic symptoms
  2. Family background
  3. Legal history
  4. Physical, behavioral, cultural, environmental, and psychosocial information
  5. Developmental history and milestones
  6. Substance abuse history
  7. Domestic violence history
  8. Physical, emotional, or sexual abuse history
  9. Demographic information
  10. Families and clients strengths and limitations
  11. Mental Status
  12. Description of problem
  13. Description of the client’s needs, wants, abilities, and desired outcomes
